A massive earthquake devastates Seoul. The city collapses, and the medical system is paralyzed, leading hospitals to order evacuation. However, dedicated surgeon Hae Seong refuses to abandon his patients and fights to save as many lives as he can. Meanwhile, Busan-based orthopedic resident Ttol Mi becomes stranded and is unwillingly pulled into the rescue by Hae Seong, who happens to be at the scene with her. As they treat the injured under extreme conditions, their clashing ideals lead to constant conflict. Yet, in the face of relentless danger, they begin to rely on each other. But as death becomes an everyday reality, their emotions waver, their beliefs collide, and their growing bond clouds their judgment. Will they protect the lives they swore to save in this shattered city?
